Due to their confidential nature, Texas crash reports are not available for online viewing by the general public in accordance with Texas law.

This page contains the crash report forms and instructions used to report all crashes. Due to TxDOT’s 10-year retention policy for crash reports, and changes to the forms over time, there are five groups of crash reports and instructions to be used by law enforcement agencies when reporting crashes. Per TxDOT retention policy, crashes that occurred on Dec. 31, 2013 or before will no longer be accepted into TxDOT’s custodial records.
TxDOT previously offered the CR-3 Alternate Texas Peace Officer’s crash report form that was formatted to include the crash report codes within the report. This form should only be used to supplement crash reports originally filed on a CR-3A.
Note: The CR-3 and CR-3 Alternate forms are for use by Texas law enforcement agencies only.
The 2023 forms and publications are to be used to report all reportable crashes occurring on or after April 1, 2023. Note: This includes supplemental reports for crashes that occurred during this time period.
